How can home care help after a hospitalization?

Q: I was told that when my dad leaves the hospital he’ll need services at home with home care. I don’t even know where to start! Help!

 

Often time’s discharges can sneak up on you, but I am so glad your doctor suggested home care as a plan for providing a safe transition home and extra support as your dad recovers from his hospital stay, whether it was from surgery or an illness. A safe transition home after hospitalization can help prevent re-hospitalization in the future.
