Freedom Home Care is there for you and your family at difficult and stressful times. We are able to provide support upon discharge from the hospital or rehabilitation facility, supervision and care for individuals with Dementia and a 24-hour Emergency Contact Service for peace of mind that you will not be alone when an emergency strikes.

HOSPITAL DISCHARGE

A safe transition home after hospitalization can help prevent re-hospitalization. Our Freedom Home Care Registered Nurses can help you return home safely by working closely with the hospital discharge planner to make sure a plan of care is set up to support you when you may be in a weakened state post hospitalization. They can also help you manage your medications. Our caregivers can pick you up from the hospital, bring you home, pick up groceries and then support your needs in the home. Some of these things may include personal cares, help with prescribed exercises to regain strength, help with mobility, cooking, cleaning and more.

EMERGENCY CONTACT SERVICE

Many people have no family members nearby to serve as an advocate for them, to care for them or to be there for them in an emergency. Freedom Home Care recognizes the importance of advocating for aging adults and offers case management as a service to those in need. Our 24-hour Emergency Contact Service offers peace of mind that you will not be alone in an emergency. Our Case Managers are Registered Nurses or Social Workers. Each month they will visit with you and become familiar with your health history and medical and personal needs. Upon an emergency situation they will accompany you to the Emergency Department or Urgent Care, stay with you as needed and communicate with any loved one that you would want to know about your condition.

REHAB DISCHARGE

A safe transition home after rehabilitation can help prevent readmission to a skilled nursing facility, keeping you where you want to be – in your home. We would start by attending your care conference to help make a plan for a safe transition home. Our Registered Nurses set up a plan of care personalized to you. They can also help you manage your medications and assess the safety of your home. Our caregivers will support you as you continue to recover by managing the day-to-day tasks that have become more difficult or tiring at home. They can also help you with any exercises you are prescribed to do to rebuild your strength.
